Our rehabilitation services and professional staff provide the supports that people with serious mental illness, physical disabilities and other developmental challenges need to be active, productive members of their communities.

Summary:
The Licensed Practical Nurse is responsible for coordinating and providing for consumers' general medical care including medications, hygiene, skills areas and staff training/education while assisting with the implementation of consumer treatment plans.

00/hour Available Shifts:
(40 hours) 1st shift, Monday through Friday, 9 AM to 5 PM
Qualifications:
Current Licensed Practical Nurse or the ability to achieve licensed status within three months of date of hire required.
Knowledge of psychotropic medications, their uses and effects strongly preferred.
Ability to provide training and supervision to staff in rehabilitation required.
Knowledge of basic principles and methods of rehabilitation required.
Ability to work creatively with and provide an empathic attitude towards people who have mental illness required.
Ability to effectively handle multiple tasks required.
Ability to take direction as well as direct others required.
Valid driver's license required.
Ability and willingness to drive agency van required.
Essential Functions:
Monitor medication of all consumers as prescribed and document on individualized medication sheets.
Report any medication errors or omissions to Residential Supervisor.
Monitor and supervise receipt, storage and distribution system for medications.
Assist visiting psychiatrist (s) and medical doctor (s) on rounds.
Notify pharmacy of new prescriptions, prescription changes and/or refills as needed.
Monitor site stock of medical first aid kits, supplies and over-the-counter medication.
Assess necessary medical equipment needs for each consumer.
Maintain contact with community medical personnel when a consumer is hospitalized or has special needs.
Carry a beeper and provide on-call coverage for medical emergencies.
Respond appropriately to emergency situations.
Advocate for clients' medical/service needs.
Serve as liaison between CareLink and other agencies, medical personnel, family members, and the community as necessary.
Record all upcoming medical appointments and pre-appointments as required.
Provide specific trainings to staff as required.
Assist clients with hygiene needs as required.
Participate in the development and implementation of individual service plans designed to improve the social and self care skills of program residents.
Assess, teach, supervise, and evaluate on an on-going basis residents' skills in the areas of daily living, meal planning, housekeeping, budgeting, vocational, social, recreational, and leisure activities.
Provide for residents' needs when they are unable or unwilling to provide for themselves.
Assist residents in the planning, initiating and participating in daily, evening and weekend social and recreational activities.
Supervise activities when appropriate.
Monitor, assess and report each resident's need for additional or replacement clothing and personal items and advise the Shift Supervisor.
Assist with kitchen and housekeeping supplies as required.
Review emergency procedures with residents on a regular basis.
Conduct emergency evacuation drills as required.
Coordinate the orientation of new residents, take inventory of all personal items upon admission, and introduce new residents to current residents.
Transport clients when requested as necessary and appropriate.
Perform daily progress notes, log book entries and other related paperwork as required.
Attend staff meetings.
Present in-service trainings and provide relevant information at staff meetings.
Participate in supervision on an on-going basis.
Provide casework services to aid clients in adjusting to their specific problems and situations.
Help coordinate, attend and participate in special agency events.
Perform related work as required.
Project a positive image to all staff.
Complete assignments within time frames.
Participate in achievement of agency objective and mission and goals.
Locate and access community medical resources as per consumer needs.
Works shifts as scheduled by supervisor.
Handle confidential information appropriately.
Performs job in a safe manner.
Reports/addresses safety concerns in a timely manner.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
